Very new arrival, but the Hummingbird from Bioshock Infinite. That thing is just incredible. In fact, while we're on Bioshock, the Big Daddies from the original were absolutely superb as well.
Oh, and the Vortiguants from Half Life. No, Striders! No, Headcrab zombies! No, wait, Combine Soldiers! No, Combine Hunters! In fact, all the stuff from the Half Life games!

Shamblers from Quake. They're huge, white, blood covered, monsters that kill you very quickly. They're pretty scary.
That and librarians from Metro 2033. They're massive, theow you around like a rag doll, and are very resistant to normal weapons.
Actually several creatures from Metro get my nomination.

The demons from Doom I might have a lock on that. Those guys scared the crap out of me back in the day. Instead I am voting on what I have the most fun fighting, and that is definitely the Doom 1 Imps. You could step into a room with a dozen of them, dodge fireballs left and right, and take them all down with the trusty shotgun. All with Belinda Carlisle on the cd player. Good times. I hated how I could not see the %*#*!! game I was playing. I could have just skipped buying the game, walking into my closet and turned out the light, and played a scary soundtrack. It would have been the same experience.

I hated how I could not see the %*#*!! game I was playing. I could have just skipped buying the game, walking into my closet and turned out the light, and played a scary soundtrack. Well the main problem for me is that at first it was really good and at the time it came out the graphics and lighting were among the best in the video game world but aside from that it was very predictable. It always goes exactly the same, you walk into a dark room, your flashlight stops working, and then you get attacked by demons from behind.

Had to try it out to see if it worked, you know  -- What bugged me the most with Doom 3 is that for some sick, stupid and completely insane reason you could not hold the flashlight in one hand while using the pistol! Why!? I say, why!?!?!? wasnt that possible??? You could hold up a rifle with your left hand so it wasnt crippled or anything. So why couldnt you hold the flashlight in your left hand?? Stupid id Software
