They attack you, at least most of them if not for a special quest or anything - One in the Dark Brotherhood for example is a vampire and he doesn't attack you, he only gives you quests
 Myself tho, I've turned into a real vampire now - and people won't even speak to me so need to find myself a way to cure the disease since I have a world to save  |
