In a three generation household, such as that of the Munsters, it avoids confusion. Lily doubtless taught the kids that the old guy is 'Grandpa' since Herman is their 'Dad'. To avoid confusion and promote consistency, particularly when the kids were very young, she joins them/sets the example by calling the old guy Grandpa. Hmmm, that makes sense I suppose.
